facility
  Definition: The physical means or equipment for facilitating the performance of an action, eg, buildings.

NOTE    Synomym to support infrastructure.

failure
  Definition: Failure is termination of the ability of an item to perform a required function ().

NOTE    Note 1: Failure is

  • an event
  • a state

Note 2: After a failure a fault will exist for some period of time.

Note 3: Faults may exist without a failure having occurred (e.g. faults as built, maintenance errors, damage).

Note 4: A failure, or the existence of a fault, in one component may cause a failure, and hence a fault, in another.

Note 5: A failure may have probability of occurrence during a given mission..

Note 6: A fault may have a probability of existing during a mission.

failure mode
  Definition: A failure mode is an event (eg failure, damage), or sequence of events, through which a particular fault, or set of faults, may arise. .

NOTE    Note 1: A failure mode may be compensated by operator action or design provision.

Note 2: A failure mode may have a probability of occurring, during a mission.

Note 3: A failure mode may only be relevant to certain plant states/mission phases.

Note 4: The same fault may result from different failure modes.

Note 5: In some other standards the term "failure mode" is used as a synonymy for fault.

fault
  Definition: A fault is an anomaly that causes loss of a required function.

NOTE    The requirement for the function, and hence the failure condition, may vary with circumstances. The same anomaly, on the same product, could be classified as a fault under one circumstance (eg full power state) and as a degradation in another (half power state).

feedback
  Definition: Feedback of information of relevance to product support.

NOTE    Includes:

  • work requests or issues which may lead to change
  • requests for identifiers for product or support elements
  • impact reports in response to change tasks
  • status reports on progress with change tasks
  • support and commissioning feedback (see own definitions).

In combination with other inputs to the configuration management activities feedback may prompt the need for a change and assist the development of a change.
